As a Community Assistance Social Worker, my primary focus is to address and alleviate food and health-related challenges within the community. My responsibilities include:

Key Responsibilities:

  • Assessing Needs: Identifying individuals and families in need of food and healthcare support through comprehensive assessments.
  • Resource Coordination: Connecting clients with local food banks, nutrition programs, healthcare providers, and other relevant services.
  • Case Management: Developing and managing individualized care plans that address both immediate and long-term food and health needs.
  • Advocacy: Advocating for clients to access essential services and work towards removing systemic barriers.
  • Educational Outreach: Providing education on nutrition, healthy living, and available healthcare resources.
  • Emergency Assistance: Coordinating emergency food and medical assistance during crises or natural disasters.
